► MANDATORY INFORMATION TO SUBMIT

Recent Photo ID , proof of participant and for accompanying person (If Less than 12 years ) First Name, Last Name, Address, State, Pincode, Contact No., FOGSI Membership No., Medical Council No., Email ID and Mobile Number of the Faculty / delegate to be filled clearly in the registration form .



► REGISTRATION GUIDELINES

  • Send the filled offine form to conference secretariat address given below along with the payment details.
  • Conference Registration fees include admission to the scientic halls, trade exhibition, inaugural function, lunch, Banquet and delegate kit.
  • Organizing committee shall not be liable in any form in case of change in date/venue due to unforeseen reasons
  • It is mandatory to submit HOD Letter/PG certicate to avail the registration in PG category
  • No Conference kit is guaranteed for spot registration


► CANCELLATION & REFUND

  • All cancellation should be made in writing and sent to Conference Secretariat Office by 28 February 2026 to
  • avail 50% refund of the registration fees after deduction of GST
  • The refund process will begin only after 30 days of the completion of conference.
  • No refund will be done if cancellation of registration is done after 1st March 2026
